Fashio Contest: OPEN, few selcted items from dance schools

Evening Program: Bollywood Performances (with Patriotic flavor requested)

We are accepting solo performances, selections are subjected to audions

Permitting ONLY Gruop performances with minimum of 4 partiacipant

I hereby agree to indemnify, save and hold harmless the India Culture Center, Houston, and the Cultural Program Committee, their memebers, agents and employees from and agaist all claims, damages, loses and expenses including reasonable attorney's fee in any lawsuit, claim or action arising out of the activities, operations, functions, facilities, equipment and locations associated with the Cultural Program and my child's participataion in this program India culture Center and/or Cultural Program Committe reserve the right to disqualify any contestant and /or contestant's family for unsportsmanlike conduct.

Reporting Times will be announced 2 weeks prior to the event date.

Reporting Location: New Stafford Civic Center

Program Rules:

Performance time slot will be based on age of performing group, content of the item, performance level & will be at the sole discretion of the organizers.
All items will be in groups ONLY, consisting minimum of 4 participants
Item Performance time will be a maximum of 3 minutes. Any item going beyond the time limit will be automatically stopped.
ALL items will be auditioned. This being celebration of Independence Day, we
request you to perform appropriate to the occasion. You performance can be Indian Classical, Regional Folk Dances or any performance with Patriotic Flavor.
Multiple items by the same group (participants) will not be permitted
There will be no duplication of the item. The song/dance number is selected will be purely on first come first served basis.
Certificated will be given to all Participants.
Parents are responsible for their children at all times.
Due to overwhelming response from the community to participate in the cultural program, there will be no last minute entries.

By signing this form I agree to abide by the rules set forth by the Cultural Program Committee and Director. In consideration of acceptance of this entry, I waive and release any and all claims for myself and my heirs against any member of the Cultural Program Committee and India Culture Center, Houston and sponsors of the Program for any injury or illness resulting for my participation in this event.
